Weight that won't move despite genuine effort is worth investigating rather than grinding harder against. Several measurable things make losing weight much harder, and none of them care how disciplined you are.

When Effort Isn't the Problem

An underactive thyroid slows your metabolism directly. Insulin resistance makes your body store fat more readily, and it's usually present years before anyone calls it diabetes. High cortisol from chronic stress or bad sleep parks fat around the middle specifically.

In men, low testosterone means less muscle, and less muscle means a lower burn rate around the clock. Fatty liver both comes along with metabolic trouble and makes it worse. Each of these has a different answer. Knowing which one you're dealing with changes the entire plan, which is the point of testing before you commit to a programme.

Who Should Consider These Tests

  • The weight hasn't moved despite consistent diet and training

  • You've gained without changing anything

  • It sits mainly around your middle

  • You're tired, cold or low alongside the weight

  • You've got PCOS, or diabetes in the family

  • You're about to start something structured

What Each Test Tells You

This page lists 8 options. Here's what each one actually measures, so you can pick instead of guessing.

Blood tests

  • Diabetes Testing (FBS + PLBS): Sugar before food and two hours after. The second number is the interesting one, because that's where trouble shows up first.

  • TSH Test: Just the brain's signal to the thyroid. It moves before anything else does, so it's the cheaper first look if you only want one number.

  • HbA1c: Your average sugar over the last two to three months. One good week won't fix this number, which is the point of it.

  • Lipid Panel: Splits your cholesterol into the good, the bad and the triglycerides. High cholesterol has no symptoms at all, which is exactly the problem.

  • Liver Function Test (LFT): Your liver handles everything you eat, drink and swallow. It puts up with a lot before it complains, so this picks up fatty liver or alcohol strain years before you'd feel anything.

  • Thyroid Function Tests: Checks the hormones that set how fast your body runs. Worth doing if you're tired, gaining or losing weight without trying, or your hair's thinning.

  • Serum Cortisol: Your stress hormone, which runs on a daily clock that night shifts and long-term stress throw off.

  • Free Testosterone: The testosterone your body can actually use, not the amount locked away. Low levels mean flat energy, low drive and muscle that won't build.

How to Prepare

  • No fasting: HbA1c, TSH Test

  • 8 to 12 hours fasting for the first sample: Diabetes Testing (FBS + PLBS)

  • Morning sample. Levels drop through the day: Free Testosterone

  • 9 to 12 hours fasting. Water's fine: Lipid Panel

  • Skip alcohol for 24 hours first: Liver Function Test (LFT)

  • Morning, before 10am: Serum Cortisol

  • No fasting. Take your thyroid tablet after the sample, not before: Thyroid Function Tests

Tell the collection team everything you're taking, prescription or not. Plenty of supplements move these numbers, and without that context your report reads wrong.

How Often to Repeat

Test at the start so you know what you're working with, then again after three to six months of treatment or lifestyle change.

Understanding Your Results

A normal thyroid result doesn't mean the effort's the problem. Insulin resistance and cortisol are separate questions and need looking at separately. Take the full panel to your doctor rather than stopping at the first normal number.

These tests point at a diagnosis. They don't make one. Read your report with a doctor who knows your history and what you're currently taking.
